4. Counter-Notification

- A statement under penalty of perjury that you have a good faith belief the material was removed due to mistake or misidentification
- Your name, address, telephone number, and email address
- A statement consenting to jurisdiction of the federal district court in your location
5. Repeat Infringer Policy
MyNamePix maintains a policy of terminating user accounts of repeat infringers in appropriate circumstances.
